On the following days the surgery will be closed at 12md for staff training and will re-open the following day at 8am. During this time the phone will go through to the out of hours service:

The practice would like to keep a register of patients who act as carers and we ask for your permission to hold your name on this register. The information you provide will be treated with the strictes confidence. Delivering IT Service
Statement of Intent for delivering IT services
Type of Service: Repeat Prescriptions, Online Booking and Cancelling of appointments.
Achieved/Not Achieved: Available Now
Notes: Patients need to Register for the online services with the practice.
Type of Service: GP to GP transfer (electronic transfer of computerised medical records between the patients registered GP)
Achieved/Not Achieved: Will be available from the 5th February 2015.
Notes: Electronic records within SystmOne* are automatically transferred to the patients registered GP if on SystmOne.
Type of Service: Summary Care Record (medications, allergies and adverse reactions)
Achieved/Not Achieved: Achieved, all patients registered at Treeton Medical Centre have a Summary Care Record created unless an opt out form is received.
Notes: Summary Care records can be accessed by GP’s, out of hours, A&E when patients attend, to facilitate safer prescribing outside the practice.
Type of Service: Patient Access to Medical Records (medications, allergies and adverse reactions)
Achieved/Not Achieved: Available from 31st January 2015.
Notes: Patients will have access to part of their medical records by registering for this online service, ID and a registration form will need to be supplied.
Type of Service: Electronic Prescriptions (electronic transfer of prescriptions to pharmacies)
Achieved/Not Achieved: Online from 19th June 2014
Notes: Will not be suitable for all patients/medications. Patients will need to nominate a pharmacy.
* SystmOne is the computer system used by the practice to record data into your medical record.
